Applicants to the PhD program are expected to have completed a master’s degree with a minimum B+ GPA (e.g. 9 out of 12, 3.33 out of 4, 77 out of 100, 16 out of 20). To facilitate comparison, please include information on the scale that applies to the GPA stated below. Please list all degrees completed or in progress. Please include electronic copies of your transcripts for all schools attended.

No application can be considered without valid GMAT or GRE results being reported. If you have not written the GMAT or GRE test, please retain this form until you can report your results. NOTE: These test results are valid for 5 years from the date of testing.

GMAT

A GMAT (Graduate Management Admission Test) score of at least 600. The GMAT test is administered by MBA.com and the scores are valid for 5 years. Please enter your highest valid scores.

GRE

A comparable GRE (Graduate Records Exam) score is acceptable in addition to a minimum of 80% quantitative reasoning score. The GRE test is administered by Educational Testing Services (ETS) and the scores are valid for 5 years. Please enter your highest valid scores.

4. English Language Proficiency


Is English your first language or did you complete a university degree at an institution where English was the language of instruction?

If your native language is not English you must provide a TOEFL or IELTS score. You are exempt from this requirement if you completed a university degree at an institution where English was the language of instruction.

If you anwser Yes and the university is outside North America, please upload a letter from the institution confirming the language of instruction.


Applicants whose native language is not English are required to furnish evidence of their proficiency in the use of the English language by achieving an acceptable TOEFL (T) or IELTS (I) score.
